-Pick-Up Form

-Vin (Preferably From Driver Side Door Only)

-Odometer

-Customer ID or ID’s (Front Only – Include Multiple ID’s In One Photo If Possible)

-Title (Front & Back)

-Separate Original Lien Release (If Applicable)

Note: It may be necessary to send photos of the front and back of an original lien release. (Example: Minnesota)
